| Registration Process:
1. Fill out the registration form on the website (blue 'REGISTRATION' button).
You will receive a confirmation email that we received your registration and are holding a spot for your team pending receipt of your deposit.
2. Send in your teams deposit to:
Ultimate Tournaments, 417 W. Prairie Ave, Wheaton, IL 60187. If you have a copy of your teams stamped USA Hockey roster you can send it in with your deposit or send it later.
3. Download the hotel reservation form from the website.
All the hotel reservations are made through us. There is a quick link for 'Hotel Reservation Form' in the bottom window of the website.
a. Fill out the form
b. Mail, email (hotels@ultimatetournaments.net) or FAX it back to us. Our FAX number is (630)206-0617.
c. Those families that don't provide credit card information on the registration form will have to call the hotel after the reservations are made to confirm their registration.
We will notify the team contact when those calls are to be made.
