All teams must be registered with USA Hockey. Teams will be required to submit a valid USA Hockey roster. Upon receipt of your registration, Ultimate Tournaments will hold a spot for your team for 5 days.

Please be sure to include your team's name and division on the check.
A nonrefundable $250.00 deposit is due at the time of application.
Balance is due (30) calendar days prior to the tournament.
If team is denied acceptance, all money will be returned
If your payment is not received within 5 days, registration will be on a first come, first served basis.
